Classic Health Check Package (Women Aged 30-40)

1. Eligibility: This package is available for women aged 30-40. Valid identification may be required upon registration.

2. Inclusions and Exclusions: This package covers only the tests and services specifically listed in the Standard Health Check-Up package description. Any additional tests, consultations, or treatments requested by the patient or deemed medically necessary by a doctor will incur separate charges. Please refer to the detailed package list for specific inclusions.

3. Validity: This package is valid for single use and must be redeemed within 3 months from the date of purchase. Extensions may be granted only under exceptional circumstances and at the sole discretion of Intercare Hospital management.

4. Appointment Booking: Advance booking is required to utilize this package. Appointments are subject to availability. Intercare Hospital reserves the right to reschedule appointments due to unforeseen circumstances. Please contact us to schedule your appointment.

5. Non-Transferability: This package is non-transferable and must be used by the individual who purchased or received it.

6. Refund Policy: Refunds are not available once the package is purchased, except under extraordinary circumstances as determined solely by Intercare Hospital. Requests for refunds must be submitted in writing with supporting documentation.

7. Results and Confidentiality: Test results will be treated as confidential and will only be shared with the patient, unless the patient provides written authorization for release to another party. Results are typically available within 3-5 business days following the completion of all tests. Some tests may have longer processing times.

8. Patient Responsibilities: Patients are responsible for adhering to all pre-test instructions provided by Intercare Hospital, including fasting or dietary restrictions. Failure to follow these instructions may affect the accuracy of test results. It is the patient’s responsibility to inquire about and understand these instructions.

9. Medical Advice: This health check-up package provides screening and assessment services but does not constitute a substitute for comprehensive medical advice, diagnosis, or treatment. Results should be discussed with a qualified healthcare professional. Follow-up consultations with a physician may be recommended or necessary based on the test results.

10. Changes to Terms: Intercare Hospital reserves the right to amend these terms and conditions without prior notice.
