RESPONSIBILITIES

Record Maintenance:
• Organize and maintain patient records, ensuring they are accurate, up-to-date, and properly filed.
• This includes paper and electronic medical records (EMR).
Confidentiality Compliance:
• Follow all legal requirements for maintaining the confidentiality of patient information (e.g., HIPAA compliance standard).
Data Entry:
• Input patient data into the medical record system, including personal information, medical history, test results, and treatment plans
File Management:
• Retrieve patient files upon request by authorized medical staff, ensuring that records are easily accessible and accurately matched to the correct patient.
Processing Documents:
• File, scan, and index documents into the electronic or paper medical records system, ensuring they are organized and categorized correctly.
Record Review:
• Review records for completeness, accuracy, and compliance with hospital and regulatory standards.
Medical Coding:
• Assist with coding procedures and diagnoses for billing and insurance purposes when required.
Coordination with Other Departments:
• Work closely with healthcare providers, administrators, and other departments to ensure smooth workflow and information sharing.
Audit and Quality Control:
• Conduct audits of medical records to ensure that they are complete and up-to-date according to standards.
Inventory Management:
• Monitor and manage the supplies and equipment needed for record-keeping.

QUALIFICATION

Qualifications:
• High school diploma or equivalent; associate’s degree in health information management or related field is preferred.
• Experience in medical records or healthcare administration is a plus.
• Understanding of HIPAA regulations and other legal requirements regarding patient information.
• Fluency in English and familiarity with the Cambodian healthcare system flow.

Required Skills & Competencies:
• Familiarity with medical record systems (e.g., Electronic Health Records or EHR), medical coding, and office software (MS Office).
• Strong organizational skills and the ability to manage a large volume of sensitive information with accuracy.
• Ability to communicate effectively with patients, healthcare professionals, and administrative staff.
• High regard for patient privacy and ethical handling of medical information.

Working Conditions:
• Medical Recorder generally work in office environments within healthcare facilities.
• They may work full-time or part-time depending on the institution’s needs.
• Interaction with patients and medical staff is common, but the role does not typically involve direct patient care.
• Ability to handle multiple tasks at once without compromising the quality of work and managing deadlines efficiently in fast-paced environments.
• Available to travel and support Intercare hospital and remote at Phnom Penh and Province
